DATA SUBJECT APPLICATION FORM FOR DATA PROTECTION

GENERAL INFORMATION

1. In accordance with Article 11 of the Personal Data Protection Law No. 6698 ("PDPL"), your rights are as follows:

• Learning whether your personal data is being processed,
• Requesting information if your personal data has been processed,
• Learning the purpose of the processing of personal data and whether they are used in accordance with their purpose,
• Knowing the third parties to whom your personal data is transferred domestically,
• Requesting correction of personal data if it is incomplete or incorrectly processed, and notifying third parties to whom your personal data has been transferred,
• Requesting the deletion or destruction of personal data in case the reasons requiring its processing disappear, despite being processed in accordance with the PDPL and other relevant laws, and notifying third parties to whom your personal data has been transferred,
• Objecting to the occurrence of a result against you by analyzing the processed data exclusively through automated systems,
• Demanding compensation for damages in case you incur damages due to the unlawful processing of your personal data.

1. Submission Methods of the Application Form in accordance with Article 13 of the PDPL: In accordance with Article 13(1) of Law No. 6698 on the Protection of Personal Data, your requests regarding these rights must be submitted in writing to our company as the data controller. Requests made by data subjects must be in writing and sent through the methods specified by the Personal Data Protection Board ("Board"). Requests within this framework must be submitted with the printed copy of this form, in person, through a notary, or by registered mail.
2. Application Methods:

• In-Person Application [The data subject personally applies by proving their identity with a document and providing information and documents about the application subject.]: The Data Subject may personally apply with identity-identifying documents and a fully completed and signed Application Form printout to "Güllübağlar Mah. Firketeci Sk. No: 2 P.K:34906 Pendik/İSTANBUL." The envelope should be labeled "Information Request within the Scope of the Personal Data Protection Law."
• Application with a Secure Electronic Signature [Application signed with a secure electronic signature and sent by Registered Electronic Mail (KEP)]: The application can be sent to hera.led@hs03.kep.tr email address. "Information Request within the Scope of the Personal Data Protection Law" should be written in the subject of the email.
• Application with Mobile Signature: The application can be sent to info@heraled.com email address. "Information Request within the Scope of the Personal Data Protection Law" should be written in the subject of the email.
• Sending the application form by using the e-mail address previously notified to the Company and registered in the Company's system: The application can be sent to info@heraled.com email address. "Information Request within the Scope of the Personal Data Protection Law" should be written in the subject of the email.

This application form has been prepared to determine your relationship with our company, to identify your personal data processed by HERA EĞLENCE VE MİMARİ AYDINLATMA SİSTEMLERİ İÇ VE DIŞ TİC. A.Ş.  (Mersis: 0461061621100001) "COMPANY," if any, in full, and to respond correctly and within the legal period to your relevant application. In order to eliminate legal risks that may arise from unlawful and unjust data sharing and, especially, to ensure the security of your personal data, the COMPANY reserves the right to request documents and information (such as identity card or driver's license copy, etc.) for identity and authority verification. The COMPANY does not accept responsibility for requests arising from incorrect information or unauthorized applications, and especially for requests arising from unauthorized data sharing.
